Springfield Pediatrics welcomes new patients. Choosing the right pediatrician for your child is a very important decision you will make. By choosing us as partners in care, you can be rest assured your care is in good hands. We very strongly recommend adherence to the vaccination schedule from the CDC/ American Academy of Pediatrics for all the patients on our service.

What to bring to your first visit?

We work hard to ensure that your child receives prompt medical care in a comfortable, child-friendly office setting. Same day visits are guaranteed for our established practice patients. We strive to accomodate all appointment requests as soon as possible. Our well designed waiting rooms with separate entry for sick and well visits are fitted for your convenience. To help expedite the first appointment, we ask that you visit our patient portal and complete all your registration securely online, or bring your child's immunization records, arrive early and be prepared to complete paperwork regarding your family medical history, your child's medical history, and insurance information.

ALL CANCELLED/MISSED APPOINTMENTS WITHOUT ADEQUATE 1-DAY PRIOR NOTIFICATION TO THE PRACTICE BEFORE THE APPOINTMENT DATE WILL BE ASSESSED A $50.00 CHARGE PER OCCURRENCE. 

Patient Forms

We encourage new patients to select the option of the patient portal on our home page, register and complete the online patient registration and review of our policies. The forms can be filled and signed electronically by you from your home computer and delivered to the pediatrician ahead of your visit and significantly make us even more efficient at providing quality care from day 1! We are also happy to still offer you the option to download, print and complete our office forms manually if desired.

What to Expect

On the day of your child's first visit to our office, we recommend arriving a few minutes early. This allows extra time for finishing up any registration forms and ensures you have plenty of time to get acquainted with our staff and office. We look forward to meeting you and fostering a real partnership in patient centered healthcare. The use of the patient portal is encouraged for all patients.
